** Job Description

Summary:

** This position is responsible for providing education and support to cancer patients and families and facilitating communication across the continuum of care. Responsibilities include management/navigation of patients, education and advocacy, and coordination of care with managing physicians. This position exists in various departments throughout Cancer Services across the Ohio Health system of hospitals and healthcare services.

* Responsibilities And Duties:

** 75%  Assesses learning needs, provides education, assesses learning achieved, and provides support and navigation to cancer patients and families.  10%  :
Participates in quality control and assurance.  10%  :
Participates in process improvement activities .  5%  :
Other duties may vary by department such as various program coordination and maintaining registries. The major duties/ responsibilities and listed above are not intended to be all-inclusive of the duties, responsibilities and to be performed by associates in this job. Associate is expected to all perform other duties as requested by supervisor.

Job Description:

** Bachelor of science ( BSN) in Nursing from a four-year college or university;
Current licensure to practice as a registered nurse in the State of Ohio. Clinical knowledge of oncology. 3 - 5 years clinical Experience in Oncology.
