Job Overview

Signify Health, a division of CVS Health®, is hiring board‑certified nurse practitioner contractors to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

• Conduct in‑home visits focused on a member’s overall health and well‑being and identify potential barriers to care.
• Complete medication and medical history review and perform a physical evaluation in the member’s home.
• Use pre‑populated, patient‑specific forms to document each evaluation.

• Earn up to $2,500 to $4,000 each week (average 6–9 visits daily, base rate $80 per completed assessment).
• Increase per‑visit rate up to 135% or 160% with the Market Multiplier (depending on working counties).
• Earn additional compensation via a pay‑per‑test model for qualified visits.
• Work Monday to Sunday, with no nights or weekends required.
• Short‑term or long‑term contracts available.
• Independent contractor (1099) with flexible scheduling.

• Airfare, hotel, and rental car coverage for eligible trips.
• Malpractice coverage.
• Availability incentives based on weekly availability.
