Altus Hospice in Georgetown, TX is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem coverage. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey.
As part of our interdisciplinary team, you will collaborate with the Clinical Director, respond to referrals, and support patients' comfort and quality of life. A current RN license and EMR proficiency are required, with on-call duties as needed.
