We are seeking a dedicated Clinical Rehab Specialist to join our team and ensure the delivery of high‑quality, efficient, and patient‑centered care. This role supports acute rehabilitation patients in a modern 30‑bed unit and works closely with a multidisciplinary medical and therapy team to achieve optimal recovery outcomes.

• Provide clinical care to acute rehabilitation patients, including those with stroke, orthopedic injuries, amputations, neurological disorders, and general debility.
• Collaborate with a highly trained therapy and nursing team to coordinate treatment plans.
• Participate in inpatient and outpatient care delivery.
• Offer hospital consults when desired.
• Work within a multispecialty group practice environment.
• Support continuity of care with hospitalists and physiatrists.
• Contribute to quality improvement and patient safety initiatives.

• Clinical rehabilitation and patient assessment.
• Multidisciplinary collaboration.
• Care coordination and documentation.
• Patient and family education.
• Evidence‑based practice.

• Acute rehab or inpatient experience.
• EMR/EHR proficiency.
• Interdisciplinary team leadership.
• Quality improvement participation.

• One of the following licenses must be obtained within 30 days of employment start date:
  • Registered Nurse (RN)
  • Occupational Therapist (OT)
  • Licensed Physical Therapist (LPT)
  • Speech‑Language Pathologist (SLP)
• Bachelor’s Degree, Master’s Degree, or Doctorate Degree in a related field.
