The Community Service Board of Middle Georgia is seeking a qualified individual to serve as our next LPN. This person will be responsible for maintaining complete, accurate healthcare records and reports. Documenting vital signs and psychiatric/medical information. Working with the medical staff to ensure that the health and safety of our clients are being best taken care of.

Responsibilities
  • Collaborate with physicians, physician assistants, and nurse practitioners through both telehealth (Zoom) and in-clinic settings to support patient care delivery.
  • Maintain complete, accurate, and timely medical records and reports for both pediatric and adult patient populations
  • Administer medications in accordance with provider orders and established clinical protocols.
  • Monitor patients for medication side effects and adverse reactions and promptly report concerns to providers.
  • Perform phlebotomy procedures as clinically indicated.
  • Obtain and document vital signs and relevant psychiatric and medical information in the electronic medical record (EMR).
  • Monitor, document, and report patient symptoms, condition changes, and treatment responses.
  • Communicate effectively with patients, families (when appropriate), providers, and interdisciplinary team members regarding care and treatment planning.
  • Complete prior authorizations and patient assistance program applications accurately and within required timelines.
  • Respond to patient phone calls and electronic messages promptly and professionally.
  • Demonstrate the ability to multitask while maintaining a high standard of patient care in both independent and team-based clinical environments.
  • Perform assigned duties independently when direct on-site supervision is not available.
  • Maintain laboratory supply inventory and ensure cleanliness and readiness of clinical work areas.
  • Notify providers of laboratory results and ensure timely scanning and documentation within the EMR.
  • Submit required monthly Infection Control, Medication Error, and HIV reports in accordance with agency policy.
  • Maintain cleanliness and appropriate documentation when operating an agency vehicle.
  • Comply with all agency policies and procedures related to agency vehicle use.
  • Maintain consistent and effective communication with supervisory staff.
  • Adhere to HIPAA regulations and always maintain strict patient confidentiality.
  • Travel to satellite clinic locations in surrounding counties as needed to support patient care operations.
  • Perform additional duties as assigned in support of clinic and organizational needs.
  • All other duties as assigned. Including but not limited to:
  • Link clients to services and resources in the community as needed
  • Collaborate with referral sources
Requirements
  • High School Diploma/GED Equivalent
  • Valid Georgia Driver’s License
  • Effective verbal and written communication skills
  • Strong interpersonal skills and the ability to work effectively with diverse communities
  • Ability to work independently and in collaboration with others
  • Experience with Microsoft 365 Office Products
  • Ability to organize, prioritize and meet deadlines accordingly
  • LPN must be licensed to practice in the state of Georgia
  • Must possess basic computer and typing skills
  • Ability to work in Adult Outpatient
