Essential Duties and Responsibilities- Plan, develop, organize and direct the Social Services Program.
- Coordinate and participate in family and patient activities designed to promote social interaction, reality orientation, and intellectual stimulation.
- Provide counseling to patients and families, including dealing with feelings about death or dying and other emotional, mental, environmental or physical limitations.
- Complete the social service portion of the MDS within 14 days of admission.
- Coordinate admissions and discharges.
- Act as liaison with social, health, and community agencies.
- Facilitate Patient and Family Councils as appropriate.
- Assist with grievance resolution when requested.
- Understand appropriate documentation for Advance Directives.
- Promote the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
- Contribute to the work of committees, work groups, project management, and other collaborative efforts of the System.
- Perform other duties as necessary to ensure the success of the System.
- Proficient in using a computer, especially Microsoft Office, Excel and PowerPoint.
- Excellent organizational and prioritizing skills.
- Effective and professional verbal and written communication skills.
- Communicates well with other associates, patients, family members and visitors, providing a warm and friendly greeting and an approachable attitude.
- Responds to expressed concerns while displaying a helpful, caring demeanor; answers questions in a professional manner.
- Minimum of a Bachelor’s Degree preferably from an approved social work program, or minimum of two years’ experience in a supervisory capacity in a hospital, skilled nursing center or other medical center.
Supervision of social services assistant(s), if necessary.
Language Skills- Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
- Ability to write routine reports and correspondence.
- Ability to speak effectively before groups of customers or associates of the organization.
- Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.
- Application of common sense understanding to carry out instructions furnished in written, oral or diagram format.
- Ability to deal with problems involving several concrete variables in standardized situations.
