Job Overview

River Brook Healthcare Center is seeking a compassionate and dedicated Physical Therapist (PT) to join our in-house rehabilitation team. We are hiring Full-Time, Part-Time, and PRN (Per Diem) Physical Therapists to provide high-quality rehabilitation services in our skilled nursing and subacute rehabilitation setting.

As a Physical Therapist, you will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions to improve mobility, strength, balance, endurance, gait, and overall functional independence. You will play a key role in helping residents recover from illness, injury, surgery, or hospitalization while enhancing safety, independence, and quality of life.

You will collaborate with an interdisciplinary team committed to resident-centered care and achieving optimal rehabilitation outcomes.

Essential Duties and Responsibilities
  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on clinical findings, physician orders, and resident goals
  • Provide skilled physical therapy interventions to improve strength, balance, mobility, gait, transfers, endurance, coordination, and functional independence
  • Evaluate and treat residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions
  • Monitor resident progress, reassess outcomes, and modify treatment plans as appropriate
  • Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility guidelines
  • Collaborate with physicians, nursing staff, occupational therapists, speech-language pathologists, dietitians, and other interdisciplinary team members to coordinate resident care
  • Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, home safety, and assistive device use
  • Participate in interdisciplinary care planning meetings, family conferences, and discharge planning
  • Recommend and train residents in the use of assistive devices and adaptive equipment
  • Maintain compliance with all professional, regulatory, and facility policies and standards
  • Promote a positive, resident-centered rehabilitation environment focused on quality outcomes and clinical excellence
Qualifications Required
  • Graduate of an accredited Physical Therapy program
  • Current Georgia Physical Therapist (PT) license in good standing
  • Current CPR certification or willingness to obtain upon hire
  • Strong clinical assessment, communication, and documentation skills
  • Ability to work effectively within an interdisciplinary healthcare team
Preferred
  • Experience in skilled nursing, subacute rehabilitation, long-term care, or inpatient rehabilitation
  • Familiarity with Medicare documentation and reimbursement guidelines
  • Experience working with geriatric and medically complex populations
  • Knowledge of evidence-based rehabilitation practices and functional outcome measures
