Job Opportunity

We are looking for a professional to work in a Home Health setting. Treatment of functional deficits, especially those that affect activities of daily living, fine motor, cognitive, sensory integration, physical mobility and developmental milestones. Patient, family, and community education. Documentation of the above information and activities. Strives for superior performance by consistently providing a product or service to leadership and staff that is recognized as ultimately contributing to the patient and family experience.

Recognizes and demonstrates understanding of patient and family centered care.

Day shift, 40 hours guaranteed perm

Requirements:

• 1 year of homecare experience
• 5-6 patients per day is the productivity
• Adult/Geriatric experience
• Well Sky/Kinnser experience
• Updated Resumé/Work History
• Pertinent Skill Checklist
• 2 Current Supervisory Professional References
