Job Overview

Folkston Park Care & Rehab Center is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our in-house rehabilitation team. We are hiring Full-Time, Part-Time, and PRN (Per Diem) Speech-Language Pathologists to provide exceptional speech therapy services in our skilled nursing and subacute rehabilitation setting.

As a Speech-Language Pathologist, you will evaluate, diagnose, and treat residents with communication, cognitive-linguistic, voice, speech, and swallowing disorders. Working collaboratively with an interdisciplinary healthcare team, you will develop individualized treatment plans that help residents regain function, improve safety, and maximize their quality of life following illness, injury, surgery, or hospitalization.

Essential Duties and Responsibilities
  • Perform comprehensive speech-language, cognitive-communication, and swallowing evaluations.
  • Develop and implement individualized treatment plans based on physician orders, clinical findings, and resident goals.
  • Provide evidence-based interventions for speech, language, cognition, voice, fluency, and dysphagia disorders.
  • Evaluate and manage swallowing disorders, including recommendations for diet modifications and safe swallowing strategies.
  • Monitor resident progress and modify treatment plans to maximize functional outcomes.
  • Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries, in accordance with Medicare, Medicaid, state, federal, and facility guidelines.
  • Collaborate with physicians, nursing staff, physical therapists, occupational therapists, dietitians, and other interdisciplinary team members to coordinate resident care.
  • Educate residents, families, and caregivers on communication strategies, swallowing precautions, cognitive rehabilitation techniques, and home exercise programs.
  • Participate in interdisciplinary care conferences, discharge planning, and quality improvement initiatives.
  • Maintain compliance with all professional, regulatory, and facility policies and standards.
  • Foster a positive, resident-centered rehabilitation environment focused on clinical excellence and quality outcomes.
Qualifications Required
  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current Georgia Speech-Language Pathologist license in good standing.
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred;
    Clinical Fellowship (CF) candidates are encouraged to apply if eligible.
  • Current CPR certification or willingness to obtain upon hire.
  • Excellent clinical assessment, communication, and documentation skills.
  • Ability to work collaboratively within an interdisciplinary healthcare team.
Preferred
  • Experience in skilled nursing, subacute rehabilitation, long-term care, inpatient rehabilitation, or post-acute care.
  • Experience evaluating and treating dysphagia and cognitive-communication disorders.
  • Knowledge of Medicare documentation and reimbursement guidelines.
  • Experience working with geriatric and medically complex patient populations.
  • Familiarity with evidence-based speech-language pathology practices.
