PT Client Support Worker; DD CAI

Days & Time
:
Up to 30 hours a month. No more than 5 days per week, but days depend on the service preferences of the individual in services.

Pay
: $15.90 per hour

• Transport and engage individual to and from community outings and events of their choice.
• Electronically document the day's activities and their reaction of the individual in the electronic medical record.
• Communicate with immediate supervisor regularly.
• Attend annual ISP meetings.
• Other duties may be assigned.

High School diploma or GED is required. Must have at least two (2) years of experience working with individuals with developmental disabilities, mental illness, and/or addictive diseases, or 2 years of providing direct care.
Must have an insured vehicle, clean driving record, and a valid driver's license.

Experience working with individuals with mental illness, developmental disabilities, and/or addictive diseases.
