Job OverviewWe are seeking a dedicated therapy professional to provide high‑quality care in a skilled nursing and rehabilitation setting. This role focuses on helping patients improve function, regain independence, and enhance overall quality of life.
Key Responsibilities- Evaluate and develop individualized care plans
- Provide hands‑on therapy and rehabilitation services
- Monitor patient progress and adjust treatment plans
- Collaborate with interdisciplinary teams
- Educate patients and families
- Maintain accurate documentation
- Active state license (PT, OT, or relevant therapy discipline)
- Experience in skilled nursing or rehab preferred
- Strong communication and patient care skills
