Role:
Physical Therapist
Responsibilities:Assess, plan, and implement rehabilitative programs to improve mobility, reduce pain, and prevent deformities. Collaborate with physicians to develop and evaluate treatment plans, establish goals, and discharge plans. Provide patient education, discharge instructions, and refer to community resources. Document evaluations, treatment sessions, and patient progress according to protocols. Act as a resource for staff, families, and physicians regarding rehabilitation services.
Qualifications & Skills:Masters in Physical Therapy (Doctorate preferred), valid PT license, BLS certification, and minimum one year of experience in rehab, LTACH, acute care, or SNF settings. Must possess effective communication, customer service focus, team orientation, and basic computer skills. Adheres to ethical practices, maintains confidentiality, and participates in professional development activities.
Key Duties:Ensure compliance with regulatory standards, organize workload efficiently, promote a positive environment, and uphold safety and infection prevention protocols. Engage in continuous learning, contribute to team efforts, and provide high-quality, patient-centered care.
