Location: South Fulton

Health Connect America seeks a Georgia-based behavioral health professional to provide community-based services to children, adolescents, adults, and families. In this client-centered role, you will complete assessments, develop treatment plans, and deliver counseling and therapeutic interventions in homes, offices, and community settings. You will collaborate with a multidisciplinary team, coordinate with schools and community agencies, maintain timely documentation, and uphold high ethical standards.

Responsibilities
  • Provide behavioral health assessments and ongoing counseling to children, adolescents, adults, and families.
  • Develop, implement, and update individualized treatment plans based on client needs and goals.
  • Deliver in-home, in-office, and community-based therapeutic services across assigned regions in Georgia.
  • Coordinate care and collaborate with schools, community agencies, and other providers.
  • Conduct risk assessments, safety planning, and crisis intervention as needed.
  • Maintain accurate, timely clinical documentation in EHR systems in compliance with regulations and payer requirements.
  • Participate in multidisciplinary team meetings, supervision, and case consultations.
  • Engage clients and families using a client-centered, trauma-informed, and culturally responsive approach.
  • Support referrals, resource linkage, and advocacy within community and social service networks.
  • Participate in ongoing training and professional development to enhance clinical skills and uphold best practices.
