The Legacy Link, Inc. ADRC Counselor is responsible for professional assessment and determination of a level of care and appropriateness for community-based services. The clients screened may or may not be Medicaid eligible or eligible for CCSP/HCBS.
The role serves as a resource to community members seeking information and referrals, adheres to HIPAA, and completes required documentation while collaborating with the ADRC Director and department team to meet productivity goals.
