Are you organized, compassionate, and passionate about helping patients navigate their healthcare needs? Primary Healthcare Centers is seeking a Referral Navigator to help connect our patients with the specialty care, testing, and resources they need.
About the RoleThe Referral Navigator serves as an important link between our patients, medical providers, and outside specialty offices. This position coordinates specialty appointments and diagnostic testing that are not provided within Primary Healthcare Centers and works directly with patients to help make the referral process as smooth as possible.
This role requires strong communication, organization, attention to detail, and the ability to provide excellent patient service.
What You’ll DoAs a Referral Navigator, you will:
- Meet with patients and assist with coordinating specialty appointments, testing, scheduling, and available financial resources.
- Communicate with specialty offices to schedule appointments and coordinate patient care.
- Send appropriate medical records, referral forms, orders, medication information, and other necessary documentation to outside providers.
- Educate patients on basic preparation requirements for diagnostic testing, including CT scans, X-rays, MRIs, and other procedures.
- Track referrals and appointments accurately within the Electronic Medical Record (EMR).
- Follow up with outside providers regarding appointments, testing results, and patient compliance.
- Receive and track testing results to help ensure continuity of care.
- Collaborate with Case Managers to assist patients with transportation, counseling, and other identified needs.
- Support Patient-Centered Medical Home (PCMH) initiatives and assist patients in developing the knowledge and tools needed to effectively manage their health.
- Assist providers in meeting applicable quality and documentation requirements.
- Participate in required PHC, OSHA, and Emergency Preparedness training.
- Maintain patient confidentiality and follow all applicable privacy requirements.
- Demonstrate professionalism, initiative, sound judgment, and the ability to work effectively as part of a team.
- Perform additional duties as assigned.
The ideal candidate will have:
- Working knowledge of Microsoft Word and Excel.
- Basic knowledge of medical terminology.
- Familiarity with medical office processes and procedures.
- Strong organizational and communication skills.
- Ability to maintain strict patient confidentiality.
- Ability to communicate professionally with patients, providers, and outside healthcare organizations.
- Referral coordination and/or medical office experience is preferred
.
This position is primarily performed in a medical office environment and includes direct interaction with patients.
The role regularly requires sitting, standing, walking, reaching, using a computer, and communicating with patients and staff. Occasional bending, kneeling, crouching, climbing, or balancing may be required. The employee may occasionally lift and/or move up to 10 pounds.
