We are looking for a dedicated patient care coordinator to join our team at Lanier Campus. The role involves ensuring high-quality patient care during the 12‑hour night shift.
Job SummaryEnsures delivery of quality patient care on the shift, coordinates patient’s plan of care, supports unit operations, and assists in staff development.
Responsibilities- Deliver quality patient care for all age groups using the nursing process.
- Coordinate patient care plans with physicians and interdisciplinary team.
- Maintain alignment with South Georgia Medical Center’s mission, vision, values, and policies.
- Assist manager with unit budgetary parameters and operations.
- Supervise, manage, and actively assist in staff development.
- Perform general unit operations as assigned.
- Current RN license in the State of Georgia.
- Current Healthcare Provider CPR certification.
- Relevant clinical experience required; leadership or supervisory experience preferred.
- Strong teamwork and individual work skills.
- Critical thinking, problem‑solving, and communication abilities.
- Ability to lead and develop others.
- Clear communication with diverse patient populations.
- Proficiency with clinical equipment, computer, and telecommunications.
- Current knowledge of patient care, leadership, and management principles.
