Nurse Practitioner; PRN

Matrix Medical Network is seeking a compassionate and autonomous Nurse Practitioner (PRN) to deliver high-quality, patient-centered care in a flexible, as-needed capacity. In this role, you will conduct comprehensive health assessments primarily in patients’ homes and community settings, as well as via telehealth when appropriate, supporting health plans and government agencies in improving outcomes and reducing costs. You will evaluate medical histories, perform focused and comprehensive physical exams, identify and manage chronic conditions, reconcile medications, and provide evidence-based education to patients and their families.

As a Nurse Practitioner with Matrix Medical Network, you will collaborate closely with an innovative and supportive clinical team while maintaining a high degree of clinical independence in the field. You will document findings accurately in electronic health records, ensure quality and regulatory compliance, and contribute to care management and risk adjustment initiatives. This position requires strong clinical judgment, excellent communication skills, and the ability to adapt to diverse home environments and patient needs.

Working on a PRN basis, you will have the flexibility to accept assignments that align with your schedule, while still playing a key role in transforming healthcare delivery. You will partner with care coordinators, physicians, and other clinicians to close gaps in care, promote preventive services, and enhance patient engagement.
