Coverage AreaWalker, Catoosa, Dade, Whitfield & Murray counties
SchedulePRN, 1-3 days/week

- Benefits may vary by employment status
- Provide 1:1 care for clients following their plan of care at all times while maintaining their dignity and independence
- Administer medications, feedings, oxygen, ostomy care as needed per client’s plan of care and documented assessments results
- Accurately document nursing actions of all care given and communicate with family, case manager, and physicians, documenting progress and outcomes for established goals and informing them of changes in clients’ medical condition and needs
- Coordinating or ordering supplies, medications, and feedings as needed to ensure adequate amounts are always on hand
- Ensuring all patient needs are met in accordance with the instructions of the physician
- LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
- Valid driver’s license, acceptable driving record, and proof of car insurance
- CPR professional certification1+ years of nursing experience preferred
- Home health care experience preferred
