Advanced Care Partners is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for medically fragile children and adults in home and school settings. This role offers the opportunity to deliver high-quality, individualized care in a more focused and meaningful environment.

Base Pay: $35/hour

Opportunity to earn up to $43/hour with applicable differentials:

• Trach: +$2/hour
• Vent: +$3/hour
• Nights (7pm–7am): +$2/hour
• Weekends: +$3/hour
• Holidays: +$3/hour

• Provide true one-on-one patient care
• Build meaningful relationships with patients and families
• Work within a supportive environment with 24/7 clinical support
• Greater consistency and autonomy compared to traditional facility settings

• Deliver skilled nursing care, including G‑tubes, tracheostomies, ventilators, and central lines
• Monitor patient condition and respond appropriately to changes
• Educate patients and families on care plans and procedures
• Collaborate with interdisciplinary team members to ensure optimal outcomes

• Active LPN license in Georgia or Nurse Licensure Compact
• A minimum of one year of LPN experience required
• Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or an equivalent accredited organization required
• Valid PPD skin test

• Ability to stand or sit for extended periods (up to 12 hours)
• Ability to bend, twist, kneel, and assist with patient mobility
• Ability to lift, push, or pull up to 50 pounds or more
• Manual dexterity for clinical tasks
