Social Circle Nursing and Rehab is currently seeking a full-time Wound Care Nurse RN/LPN (Monday-Friday). As part of the Wound Care Team, the wound care nurse participates in assessing and evaluating the clinical needs of residents with wounds, and works with the resident and the rest of the care team in planning and implementing care that addresses the needs. This work is carried out in support of the mission of Social Circle Nursing and Rehabilitation Center.
Responsibilities- Conduct comprehensive wound consultations including detailed wound assessments and recommendations for interventions and care planning.
- Conduct wound assessments for residents and complete all associated documentation, tracking, and care planning activities.
- Ongoing monitoring of all assigned residents with wounds.
- Assess resident/caregiver knowledge of wound prevention and treatment and provide education as needed.
8:00am - 4:30pm
Wound care experience is a must!
