The Family Nurse Practitioner (FNP) is an advanced practice registered nurse who provides comprehensive, evidence‑based primary and episodic care to patients across the lifespan in alignment with the mission, values, and clinical standards of East Houston Medical Center (EHMC). The FNP functions as an integral member of amultidisciplinary team, promoting exceptional patient outcomes, service excellence, and a culture of safety for the diverse East Houston community.
Key Responsibilities- Deliver high‑quality primary and urgent care, including comprehensive histories,physical examinations, and age‑appropriate screenings for patients from infancy through older adulthood.
- Diagnose and manage acute and chronic health conditions (e.g., hypertension,diabetes, minor injuries, infections) and develop individualized, evidence‑based treatment plans.
- Prescribe and manage medications, durable medical equipment, and other therapeutic interventions in accordance with Texas law, EHMC policies, and current clinical guidelines.
- Order,perform, and interpret appropriate laboratory tests, imaging studies, andother diagnostics; refine plans of care based on clinical findings and patient response.
- Provide proactive health promotion, disease prevention, and risk‑reduction counseling, including immunizations and routine screening programs tailored to the East Houston population.
- Educate patients and families on diagnoses, treatment options, self‑management strategies, and lifestyle modifications that support long‑term wellness and health equity.
- Coordinate care and make timely referrals to EHMC specialists, ancillary services,and community resources to optimize continuity of care and patient experience.
- Maintain meticulous, timely, and compliant documentation in the electronic health record, reflecting clinical reasoning, interventions, and patient response.
- Collaborate closely with physicians, nurses, case managers, and allied health professionals to support integrated care pathways, throughput, and quality metrics across EHMC.
- Upholdand model EHMC’s standards for patient safety, infection prevention,regulatory compliance, and service excellence in every patient and family encounter.
