Location: Stonecrest
Ennoble Care is seeking an experienced Per Diem Nurse Practitioner to provide face-to-face assessments for hospice patients in the Gwinnett and DeKalb Counties, GA region. You will work under physician supervision to care for a diverse set of patients with chronic diseases and complex needs.
You will review schedules, ensure timely F2F encounters, and coordinate with the hospice IDT while documenting objective findings and prognosis indicators to support eligibility.
