Undisclosed, Providence Care has merged with Your Health, and we’re expanding our team. We’re currently hiring for a Visiting Nurse to provide mobile primary care in the Blue Ridge area.

This is a full-time opportunity with flexibility. Our mobile approach allows you to provide care directly in patients’ homes and communities rather than being tied to a traditional office or hospital setting.

• Looking for someone with a Hospice heart and passion for providing end of life care
• Provide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDG
• Responsible for overseeing patient plan of care with the collaborating physician
• Provide supervisory visits and oversight for LPN and CNAs
• Provide education to patients, families, and facility staff
• Provide oversight to Aides

• Must have a RN license unincumbered in the state you are applying
• Reliable transportation and valid driver's license, clean MVR
• Experience in primary care, home health, or hospice is preferred.
