You will coordinate palliative services, support the Nurse Practitioner and Medical Director, schedule visits, and ensure clear communication of goals of care.
Top qualifying candidates hold an active RN or LPN license, at least three years of nursing experience, with hospice/palliative care preferred; CHPN is a plus.
