Job Details
- American Heart Association BLS certification required for all direct patient care personnel.
- License/Certificate verification must be dated within 30 days of start.
- Fit Test required.
- Trinity 10 Panel Drug Screen required within 30 days of start date.
- Completion of Trinity Code of Conduct Form.
- Completion of Trinity I9 form.
- Completion of Trinity Confidentiality of Patient and Proprietary Information and Information System Security Agreement.
- Completion of Trinity Integrity and Audit Services Form.
- Trinity Respirator Evaluation with physician signature required.
- Proof of COVID-19 vaccine or approved declination/exemption.
- Proof of MMR vaccination or IgG titer.
- Hepatitis B vaccination series required prior to start.
- Influenza vaccine proof or declination (optional).
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required.
- Proof of Varicella vaccination or IgG titer.
- Physical/Health Statement from exam within last year.
- Trinity Profile including resume, skills checklist, and professional references.
- Trinity Conflict Of Interest Disclosure Form.
- Trinity Criminal Background Check within last 30 days.
- Trinity Education Verification.
- Trinity EPLS report within last 30 days.
- Trinity GSA/OIG report within last 30 days.
- Trinity Joint Commission Testing (CORE Exam within a year).
- Trinity NSO report within last 30 days.
- Trinity System Access Form to be completed one week before start.
- Trinity Skills Checklist for clinical contractors.
- Trinity OFAC completion within 30 days of start.
