Targeted Case Manager

Responsibilities

• Provide case‑management services and assist individuals in gaining access to needed medical, social, educational, and other services.
• Conduct culturally appropriate assessments and review or complete a care plan on a periodic basis.
• Maintain strict confidentiality at all times.

• Bachelor’s degree in social work, sociology, psychology, or counseling from an accredited university or college.
• At least two years of case‑management experience in social service or health‑related fields.
• Experience with data gathering, reporting, and conducting assessments.
• Successful completion of a criminal background check with Federal, Tribal, and State Law Enforcement and of a pre‑employment alcohol and drug screening.
