• Provides case management services, home visits, and networking with agencies in assigned areas.
• Creates individualized action plans.
• 50% – Responsible for the implementation of seasonal initiatives such as Christmas Distribution and components of School Readiness Program.
• 30% – Responsible for keeping required records and providing required reports for the program.

• High School Diploma or Equivalent
• In General Studies
• 18 Months Additional Training in related field (Preferred)
• 1 or more years of successful experience in a related field or a combination of education and experience (Preferred)
• Possess a Valid Kentucky Driver’s License and Maintain a Safe Driving Record – Required
• CAP Driving Certification (or ability to obtain within first 90 days) – Required
• In order to drive a personal vehicle on CAP business, must obtain and maintain a policy of motor vehicle insurance coverage on a privately‑owned vehicle with insurance coverage amounts of $50,000 bodily injury per person per accident; $100,000 bodily injury for all bodily injuries per accident; and $50,000 property damage per accident.
