Essential Job Functions- Provides case management services, home visits, and networking with agencies in assigned areas.
- Creates individualized action plans.
- 50% – Responsible for the implementation of seasonal initiatives such as Christmas Distribution and components of School Readiness Program.
- 30% – Responsible for keeping required records and providing required reports for the program.

- Possess a Valid Kentucky Driver’s License and Maintain a Safe Driving Record – Required
- CAP Driving Certification (or ability to obtain within first 90 days) – Required
- In order to drive a personal vehicle on CAP business, must obtain and maintain a policy of motor vehicle insurance coverage on a privately‑owned vehicle with insurance coverage amounts of $50,000 bodily injury per person per accident; $100,000 bodily injury for all bodily injuries per accident; and $50,000 property damage per accident.
- Ability to reach, push, pull, bend, stoop, stand, walk
- Lift 50 lb from floor to waist
- Lift 50 lb from waist to shoulder
- Lift 25 lb from shoulder to overhead
- Carry 50 lb for 25 ft
- Lifts/moves items with unknown weight or that are awkward shapes and sizes with assistance
- Have good visual acuity with corrective lenses
- Have good hearing capabilities
