Northeast Georgia Health System is seeking a Case Management Social Worker to provide medical social work and case management within the hospital setting. You will assess social, psychological and financial needs, collaborate with patients, families and the care team, and implement plans addressing identified needs.
You will work across neonatal to geriatric age groups, ensure compliance with safety guidelines, and provide cross-coverage for social workers as needed, including weekend rotations.
