Job Summary:
Under general supervision, the Case Manager provides comprehensive care coordination, service linkage, advocacy, and case management services for individuals receiving behavioral health, substance use disorder, housing, recovery, crisis, and community-based services. This position coordinates behavioral health, medical, social, vocational, housing, and community support services to improve access to care, increase engagement in treatment, and support positive health and recovery outcomes. The Case Manager utilizes a person-centered, trauma-informed, and recovery-oriented approach to service delivery while promoting consumer self-sufficiency, community integration, and wellness.
Essential Functions:- Coordinates behavioral health, medical, social, vocational, housing, and community support services for assigned individuals.
- Conducts service needs assessments and assists with treatment planning activities.
- Facilitates referrals, service linkages, and transitions of care.
- Assists consumers in accessing benefits, healthcare services, housing resources, employment services, transportation, and community supports.
- Engages consumers and natural support systems in treatment and recovery processes.
- Collaborates with healthcare providers, behavioral health professionals, community agencies, and other stakeholders.
- Monitors service delivery to ensure continuity of care and achievement of treatment goals.
- Coordinates discharge planning and aftercare services.
- Provides advocacy and support to consumers in navigating healthcare and social service systems.
- Assists consumers in identifying and overcoming barriers to treatment, recovery, and community stability.
- Provides crisis support and service coordination when appropriate.
- Maintains compliance with documentation, Medicaid, DBHDD, payer, and agency requirements.
- Maintains confidentiality of consumer information in accordance with HIPAA and organizational policies.
- Completes required documentation accurately and within established time frames.
- Maintains assigned productivity standards and performance expectations.
- Participates in interdisciplinary treatment team meetings and service planning activities.
- Performs related duties as assigned.
Skills:
Knowledge
- Knowledge of behavioral health, substance use disorder, recovery, and community support service principles.
- Knowledge of care coordination, case management, and service planning practices.
- Knowledge of crisis intervention techniques and community resource systems.
- Knowledge of person-centered and trauma-informed service delivery approaches.
- Knowledge of Medicaid, DBHDD, and behavioral health service requirements.
- Knowledge of healthcare, social service, vocational, educational, housing, and community resource systems.
- Knowledge of consumer rights, confidentiality requirements, and applicable regulations.
- Knowledge of treatment planning, discharge planning, and continuity of care practices.
- Ability to coordinate services across multiple systems of care.
- Ability to advocate effectively for consumers and families.
- Ability to establish and maintain collaborative relationships with providers, agencies, and community partners.
- Ability to communicate effectively both verbally and in writing.
- Ability to maintain timely, accurate, and professional documentation.
- Ability to prioritize, organize, and manage multiple cases simultaneously.
- Ability to maintain confidentiality and professional boundaries.
- Ability to identify consumer needs and coordinate appropriate services and supports.
- Ability to resolve barriers to treatment and service engagement.
- Ability to work independently while functioning effectively within a multidisciplinary team.
- Ability to utilize electronic health records and related software applications.
Care Coordination
- Coordinates behavioral health, medical, housing, social, vocational, and support services for assigned consumers.
