Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:

Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Head Start, and Child Development Center.
Clinical Health Services:
Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV;
Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
Supportive Health Services:
Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

Areas of Responsibility:

Support the goals, philosophy, and objectives of Community Action Health Center (CAHC). Work collaboratively with the psychiatrist to provide patient assessment and diagnosis, formulate treatment plans, prescribe medications, order laboratory tests, interpret test results, provide patient education, prepare medical records, and refer patients for inpatient care in accordance with statues, regulations and protocols regulating the profession. Respectful of the confidentiality of the patients, co-workers, and clinic.

Major Duties:

Provide behavioral health patient assessment, diagnosis, and treatment plans in accordance with statutes, regulations and protocols regulating the profession.

Promote positive health behaviors and self-care skills through education and counseling.

Collaborate with physicians, therapists and other health professionals as needed.

Consult, refer and collaborate with other disciplines involved in the delivery of total patient care.

Document accurately all assessments, interventions, client/family responses, medication dispensed or prescribed, procedures, treatments, and other data in medical record.

Utilize the resources of CAHC and the surrounding area community.

Maintain a clean, safe, and efficient clinic environment for clients, visitors and staff.

Demonstrate evidence of continuing professional growth by involvement in peer review, the review of protocols and procedures and participate in quality improvement within the clinic.

Attend CAHC meetings and in-service offerings and participate in assigned committees.
