General Job Summary

Provide assistance with activities of daily living and independent activities of daily living according to each resident’s negotiated service agreement. Administer and assist with medications according to the physician’s orders and negotiated service agreement. Document and report any adverse health reactions residents may have. Contact and follow up with physician offices when directed by the Director of Healthcare. Take physician’s orders over the phone, properly noting them and leaving notification of the new order for the appropriate staff and Director of Healthcare.

Notify pharmacy of new physician orders when appropriate. Check new pharmacy deliveries, making sure the medications match the physician order. Follow universal precautions in providing care to residents. Provide first aid to residents within license and training when required. Participate in staffing and scheduling when directed to by the Director of Healthcare. Act as the shift supervisor for the resident care staff working the same shift.

Assist Director of Healthcare in taking emergency after hours and week‑end calls. Assist in other nursing duties within license requirements as assigned by the Director of Healthcare. Assist other direct care staff in providing care meeting the quality and standards set by the management of the facility.

Scope of Job Tasks
  • Provide assistance to residents according to their needs, with professionalism and dignity, address residents with respect.
  • Assistance may be supervision, partial assist, or complete assist depending on the resident’s needs as specified in the negotiated service agreement.
  • Prepare medications for administration to residents in a safe manner, according to the five rights to ensure that the proper resident receives the proper medication.
  • Record medications administered to residents in the Medication Administration Record.
  • Follow up with the physician or Director of Healthcare regarding any discrepancies in medications or physician’s orders.
  • Assist or remind residents of medication according to their negotiated service agreement if resident is allowed to self‑administer.
  • Contact and follow up with physicians’ offices. Take physician orders over the phone, note them properly, provide information to appropriate staff and Director of Healthcare.
  • Notify pharmacy of new physician orders and document form of events that occur.
  • Document any occurrence during the shift, report to the next shift any occurrences during the shift.
  • Remind residents of meal times, as well as activities and events, assist them if needed.
  • Act as the charge person in the event of an emergency evacuation drill if the Director of Healthcare or Executive Director are not in the facility.
  • Answer resident calls on the code alert system within the appropriate period of time and provide necessary assistance.
  • Provide first aid to the residents and other nursing duties within license and training as delegated by the Director of Healthcare.
  • Contact 911 in case of a medical emergency and instruct other staff members of their duties, making the resident(s) involved comfortable.
  • Provide services such as medication administration, blood sugar tests, minor dressing change, etc.
  • Act as the shift supervisor for the resident care staff working the same shift as directed by the Director of Healthcare.
  • Should be a motivator for other staff members showing enthusiasm and setting a good example, being a good role model.
  • Assist the Director of Healthcare in taking emergency calls after hours and on week‑ends.
  • Should suggest changes that would promote further efficiency in the workplace to the Director of Healthcare for consideration.
Education

Licensed as a Licensed Practical Nurse in the appropriate state of the facility. Must be able to read, write, have adequate vision and follow oral and written instructions.

Work Experience

Work experience in senior housing or other related healthcare field. Will receive additional on‑the‑job training on Germantown Plantation’s philosophy of care, assisted living, and facility policies. Ability to work cooperatively with others.

Physical Demands

Stands and walks most of the working day. Must stoop and reach to heights 5 feet or above and lift up to 50 lbs. Assist residents in weight transfer support from sitting to standing position. Must be in good physical health, free from communicable disease. Neat in appearance. Must be able to work rapidly and to hear, talk, read, and have adequate vision.

Must be flexible. Have hand dexterity and ability to assist residents with medications and dressing.

Supervision

Nursing Supervisor, Director of Healthcare, Executive Director

Working Conditions

Works in a clean, well‑lit and ventilated environment. Change in environment temperature may occur due to individual heating and cooling systems.

Dress Attire

The facility dress code is required and consists of well‑pressed colorful scrub suits.
