Position: Home Health RN Case Manager - Giddings and Surrounding Area
Overview:

The Registered Nurse (RN) Case Manager provides skilled nursing care and case management to patients in their homes. This role involves assessing patient needs, developing and implementing individualized care plans, coordinating care with the interdisciplinary team, and ensuring compliance with state and federal regulations. The RN Case Manager serves as a liaison between patients, families, physicians, and other healthcare providers to ensure high-quality, compassionate care.

Essential Functions:

· Conduct comprehensive patient assessments.
· Develop individualized care plans in collaboration with patients, families, and healthcare providers.
· Provide skilled nursing care in accordance with the plan of care.
· Monitor patient condition and report changes to the physician and interdisciplinary team.
· Administer medications and treatments as prescribed.
· Promote infection control and safety practices during patient care.
· Educate patients and caregivers on disease management, medications, and care techniques.
· Coordinate transitions of care between hospitals, rehabilitation centers, and home settings.
· Advocate for patients to ensure access to appropriate services and resources.
· Document all case management activities accurately and timely.
· Collaborate with interdisciplinary teams to optimize patient care.
· Participate in on-call rotation.
· Maintain compliance with nursing standards, ethical guidelines, and legal regulations.
· Ensure compliance with agency policies, procedures, and regulatory requirements.
· Participate in in-service training and quality improvement activities.

Job Qualifications:

Education:

Graduate of an accredited nursing program (associate or bachelor’s degree in nursing).Licensure:
· Current Registered Nurse (RN) license in the State of Texas.
· Current driver’s license.

Experience:

Minimum of 2 years of clinical nursing experience; case management experience preferred.

Skills:

· Strong knowledge of nursing standards and Texas Board of Nursing regulations.
· Excellent communication, organizational, and critical thinking skills.
· Ability to work independently and collaboratively in a multidisciplinary environment.
· Proficiency in electronic health records and case management software.

Environmental and

Working Conditions:

· Works in patients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases.
· Requires the ability to travel locally and work a flexible schedule.
· May be exposed to unpleasant weather conditions.
· Work hours may vary depending on patient needs and organizational requirements.

Physical and Mental Effort:

Requires prolonged standing and walking, the ability to lift up to 50 pounds, and the ability to move patients. Must be able to manage stressful situations calmly and courteously and make quick decisions to meet patient needs.
