Admissions Nurse

Join Life Care Centers as an Admissions Nurse supporting seniors through compassionate, coordinated transitions into care. Responsibilities

• Perform comprehensive admission assessments and nursing evaluations for new residents
• Review and reconcile medical histories, medications, and provider orders on admission
• Coordinate with physicians, therapy, and social services to ensure smooth, timely admissions
• Educate residents and families on care plans, services, and facility procedures
• Document assessments, orders, and care plans accurately in the electronic health record
• Ensure compliance with state, federal, and facility policies during the admission process
• Monitor early clinical status and communicate changes promptly to the care team
• Support discharge and transfer planning related to admission needs
