We are seeking a Clinical Pharmacist to join our pediatric emergency department at the East Valley Campus. The role involves 10‑hour shifts on a 7 on/7 off rotating schedule with evening shift differential, a starting bonus up to $10,000, referral bonuses, and relocation assistance. The pharmacist will oversee the medication management process for patients from neonate to young adulthood, ensuring compliance with laws and regulations, and will provide comprehensive pharmacy services within the framework of the Arizona State Board of Pharmacy.
PositionDuties
- Review and manage medication orders for therapeutic appropriateness based on patient characteristics and clinical information, and verify orders accurately and timely.
- Screen for clinical alerts (allergies, interactions, duplications, ADRs) and intervene as necessary.
- Problem‑solve medication order issues, contacting prescribers and recommending alternative therapies.
- Comply with Pharmacy and Therapeutics Committee directives related to formulary management.
- Document clinical interventions to meet cost‑containment and evidence‑based goals.
- Prepare and dispense sterile and non‑sterile medications in compliance with policies, laws, and quality standards.
- Provide clinical pharmacy skills in assessing and recommending medication therapy to medical staff and other providers.
- Participate in decentralized pharmacist‑assigned duties when applicable.
- Assist in developing guidelines, policies, and monitoring strategies for medication use.
- Conduct ongoing education and training of pharmacists, residents, interns, technicians, and students.
- Directly supervise technical, intern, and support staff to ensure accuracy and workflow efficiency.
- Act as liaison between pharmacy and medical and nursing staff for medication management issues.
- Participate in hospital and department quality and safety initiatives.
- Communicate problems and patient needs to incoming staff for continuity of care.
- Process incident, adverse drug reaction, and controlled substance documentation reports.
- Participate in drug use evaluation, quality assurance, and performance improvement activities.
- Maintain investigational drug study records and documentation.
- Provide evidence‑based medication education and information to patients, caregivers, and staff.
- Offer counseling for outpatient prescriptions and target medication information for inpatient and outpatient areas.
- Perform miscellaneous job‑related duties as requested.
- Master’s or Doctorate Degree (Pharm.D.) in Pharmacy or an equivalent accredited program; graduation within 6 months acceptable.
- Completion of an ASHP‑accredited PGY‑1 pharmacy residency program (preferred).
- Experience with medication information systems and automated pharmacy equipment (required).
- One year of experience in a hospital pharmacy practice or equivalent (preferred).
- One year of experience with a pediatric patient population or equivalent (preferred).
- Proficiency in MS Office Suite or equivalent computer software (required).
- Problem‑solving and collaboration skills, demonstrating empathy, teamwork, and constructive feedback (required).
- Current licensure as a pharmacist in Arizona or eligible by reciprocity or examination (required).
- Current Basic Life Support (BLS) and Pediatric Advanced Life Support (PALS) certification;
Advanced Cardiac Life Support (ACLS) for healthcare provider preferred (BLS required).
