We are currently seeking a full-time Discharge Supervisor for our behavioral healthcare facility in Casa Grande, AZ

In this role you will be actively involved in patient discharge planning from time of admission and ensure that family members, enrolled providers, and other identified supports are actively engaged and involved in the process. The Discharge Supervisor is responsible for overseeing all aspects of discharge for patients receiving services in Crisis, Inpatient, or Transition Point programs. The Discharge Supervisor attends 4-hour staffing and daily huddles with the multidisciplinary team to identify any potential discharge barriers and provide feedback on current discharge plan. The Discharge Supervisor ensures the discharge transition plan accurately reflects the discharge plan including placement name, address, outgoing referral and follow up appointments.
The Discharge Supervisor works directly with the Clinical Manager, Clinical Director, and Operations team to ensure that discharge planning and services are effective and efficient in addressing patient/participant and community needs as well as are delivered in accordance with Clinical Policy and Procedure, Arizona Administrative Code, ADHS/DBHS Provider Manual, Code of Federal Regulations, and contract requirements.

- Bachelor's Degree in related field required, Master’s Degree preferred.
- 1-3 years of full-time Behavioral Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
- Fingerprint clearance card required.
- Current AZ Driving License.
- Clear 39-month motor vehicle record.
Pay Details: 50,000.00 to 60,000.00 per year
