Key Responsibilities:
Serve as the primary point of contact for prospective learners, families, referral sources, and community partners throughout the intake and placement process.
Coordinate learner inquiries, intake activities, funding verification, Academy placement assessments, and enrollment processes from initial contact through service initiation.
Coordinate departmental assessments, orientations, and follow-up activities to facilitate timely learner onboarding and service implementation.
Provide exceptional customer service by responding to inquiries, addressing questions and concerns, and maintaining ongoing communication with learners and their representatives.
Coordinate new, modified, and ongoing learner service requests in accordance with organizational policies, procedures, and funding requirements.
Manage learner intake documentation, waitlists, contact records, tour scheduling and facilitation, inquiry forms, and related tracking systems to ensure accurate and timely records.
Respond to and coordinate Requests for Service, website inquiries, phone inquiries, emergency vendor requests, and referrals in a timely and professional manner.
Coordinate Community Partnership orientations, required documentation, and communication between learners, providers, instructional staff, and organizational departments.
Coordinate and attend new learner and ongoing quarterly ATC monitoring activities, including scheduling, database management, documentation, family and provider communication, and regulatory recordkeeping.
Maintain accurate, confidential, and compliant learner records in accordance with organizational policies, funding requirements, and applicable regulatory standards.
Maintain comprehensive knowledge of organizational programs, services, policies, procedures, and funding sources to accurately support learners, families, and referral partners.
Collaborate with interdisciplinary teams to support efficient learner onboarding, service delivery, and continuity of care.
Perform other duties as assigned.
Minimum Qualifications:
Must be at least 18 years of age.
Valid Level One Fingerprint Clearance Card (or the ability to obtain one) and a signed Criminal Record Disclosure Statement.
Provide three (3) professional references (forms provided).Current CPR and First Aid certification (original certificates must be provided for copying and personnel file).Proof of training and demonstrated proficiency in Arizona Administrative Code (A.A.C.) Title 6, Chapter 6, Article
9.Minimum of two (2) years of experience working with individuals with developmental disabilities.
Understanding of developmental milestones, social skill deficits, and maladaptive behaviors.
Knowledge, skills, and experience necessary to successfully perform the essential functions of the position.
Self-motivated with exceptional organizational and time management skills.
Strong leadership abilities with experience training and supporting others.
Strong problem-solving and project management skills with exceptional attention to detail.
