Center Well Home Health is seeking a licensed Physical Therapist to provide skilled PT at patients' homes in Arizona. You will assess, plan, and deliver individualized rehabilitation programs to improve strength, mobility, and independence.
You will coordinate with physicians, caregivers, and the care team to monitor progress, document care, and ensure compliance with the patient’s plan of care. A minimum of one year experience and a valid PT license are required.
