As a Physical Therapist in home health care, you’ll design individualized care plans, provide hands‑on therapy, and collaborate with a supportive team, all with scheduling flexibility that fits your lifestyle.

• Flexible scheduling to support work‑life balance
• Autonomy in managing your day and patient care

• Develop and update individualized care plans
• Provide in‑home physical therapy services per physician’s plan of care
• Assess functional status, mobility, and home environment
• Educate patients and families on therapy techniques and equipment
• Maintain accurate documentation and communicate progress to physicians

• Licensed Physical Therapist in the State of Arizona
• Degree from a CAPTE‑accredited program; passed state‑approved exam
• Minimum 2 years’ experience (home health preferred)
• Current CPR certification
• Fingerprint Clearance Card (or immediate ability to apply)
• Valid driver’s license, insurance, and reliable transportation
