As a Physical Therapist in home health care, you’ll design individualized care plans, provide hands‑on therapy, and collaborate with a supportive team, all with scheduling flexibility that fits your lifestyle.
- Flexible scheduling to support work‑life balance
- Autonomy in managing your day and patient care
- Rewarding work that helps patients regain independence
- Develop and update individualized care plans
- Provide in‑home physical therapy services per physician’s plan of care
- Assess functional status, mobility, and home environment
- Educate patients and families on therapy techniques and equipment
- Maintain accurate documentation and communicate progress to physicians
- Licensed Physical Therapist in the State of Arizona
- Degree from a CAPTE‑accredited program; passed state‑approved exam
- Minimum 2 years’ experience (home health preferred)
- Current CPR certification
- Fingerprint Clearance Card (or immediate ability to apply)
- Valid driver’s license, insurance, and reliable transportation
