About the Role
We are seeking a compassionate, dependable, and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide in‑home patient care near Hunters Point, AZ
. The role involves delivering high‑quality nursing services in accordance with individualized plans of care.
This is a 1099 independent contractor opportunity with an average of 8 hours of nursing care per month, offering a flexible schedule.
Employment NoteEmployment is contingent upon successful completion of a background check.
Responsibilities- Execute physician‑prescribed plans of care.
- Complete required documentation in the system of record.
- Administer prescribed medication, treatment, and therapies.
- Conduct patient assessments and coordinate care.
- Perform activities associated with daily living.
- Monitor vitals and GI intake and output.
- Assess skin integrity and administer wound care.
- Monitor for changes in the patient’s condition such as weight changes, self‑care abilities, and disease progression.
- Communicate with families, physicians, case managers, and other prescribed individuals regarding patient health.
- Provide education and training to the patient, the family, and the Home Health Aide.
- Work one‑on‑one in the patient’s home to create a comfortable environment.
- Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
- Provide other personal care services as necessary to meet the patient’s needs.
- Graduate of an accredited college or university;
Associate or Baccalaureate School of Nursing. - Current state or compact RN or LPN license.
- Current CPR certification.
- Valid TB test and physical examination.
- Required liability insurance.
- Good interpersonal skills.
- Hand‑eye coordination and manual dexterity.
- Ability to stand or walk for prolonged periods, lift up to 50 lbs, and move patients.
- Ability to operate under stress and make quick decisions.
- Reliable transportation and willingness to commute to patients’ homes.
- Ability to provide basic medical management up to medically fragile end‑of‑life care.
- At least 1 year of experience as an RN/LPN in a clinical or home‑health setting.
- Ability to assist with two‑person lifts.
