Performs a nursing assessment on all patients within 8 hours of admission. The data collected forms the basis of an individualized plan of care. Identifies teaching needs and goals related to the diagnosis of each patient from the nursing admission assessment and by collaborating with the patient/family and the Interdisciplinary team.
Provides patient information and status in a timely manner for the Initial Team Conference and for Re‑evaluation Team Conferences on a weekly basis. Is responsible and accountable for accurate, succinct and timely documentation of all assessments, interventions and patient’s response in the medical record in accordance with policy.
Coordinates patient care in collaboration with a wide array of healthcare professionals. Facilitates the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness. Ensures compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
Performs physical exam and health histories. Provides health promotions, counseling, and education. Administers medications, wound care, and numerous other personalized interventions.
Directs and supervises care provided by other healthcare professionals. Is accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel. Recognizes adverse signs and symptoms and quickly reacts in emergency situations.
Communicates and collaborates with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization. Makes referral appointments and arranges specialty care as appropriate. Performs nursing services identified in the TO.
Conducts research in support of improved practice and patient outcomes. Prioritizes activities and plans for effective use of time and staff in meeting patient needs. Performs other duties as requested.
Qualifications- Associate Degree of Nursing
- Graduated from an accredited college or university by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
- One year of experience within the last two years as a registered nurse
- Current, full, active, and unrestricted license to practice as a Registered Nurse
