As a Home Health LPN, you’ll be at the heart of patient care, bringing clinical expertise and compassion directly into the home.

• Implement plans of care initiated by the registered nurse
• Provide accurate and timely documentation consistent with the plan of care
• Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s)
• Participate in coordination of home health services, appropriately reporting identified needs for other disciplines
• Participate in personal and professional growth and development
• Perform other duties as assigned by the registered nurse

• Current LPN license in the State of Arizona
• Valid driver’s license and reliable, insured vehicle
• Current Fingerprint Clearance Card or the ability to apply
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and make a real difference
