Gentiva Hospice is seeking a compassionate Hospice Licensed Practical Nurse (LPN) to join our team providing hands-on care in patients’ homes, assisted living, or facility settings.
You will work with an interdisciplinary team, assess patient and family needs, and contribute to individualized care plans while reporting changes promptly to the RN and care team. This safety‑sensitive, on‑site role requires CPR, a valid driver’s license, and reliable transportation.
