Role SummaryFull‑time Physical Therapist Case Manager in Santa Clara County (Gilroy, Morgan Hill, and surrounding cities). You will evaluate and treat patients in their homes, develop individualized care plans, supervise Physical Therapy Assistants (PTAs), and coordinate with physicians and interdisciplinary teams to help clients live safely and productively in their homes.
Base Pay Range$120,000 / yr – $135,000 / yr plus 10K annual bonus. Eligible for quarterly incentive bonus and 401k (up to 4% match).
Responsibilities- Conduct home evaluations and develop detailed care plans.
- Supervise PTAs per agency and regulatory standards.
- Help patients with mobility, movement, and independence.
- Communicate and coordinate with patients’ physicians to ensure quality care.
- Evaluate and report patients’ progress to MD.
- Assist with documentation and administrative tasks.
- Current unrestricted CA Physical Therapist License.
- Current BLS/CPR Certification.
- Valid CA Driver’s License.
- Experience in home health care highly preferred.
- Experience with high‑acuity patients a plus.
- Experience working with an interdisciplinary team desired.
- Case Management fundamentals highly desired.
- Self‑directed, empathetic, critical thinker.
- Outstanding interpersonal, leadership, and organizational skills.
