Family Nurse Practitioner – Mobile Health Program

Conduct physical exams, diagnose and treat acute and chronic illnesses, and provide routine primary care
Take medical histories and perform physical examinations
Order and interpret diagnostic tests
Develop and manage treatment plans for chronic conditions including diabetes, hypertension, and asthma
Maintain accurate and timely patient records using EHR systems
Assist with opening and closing the clinic
Manage inventory, patient follow-up, insurance verification, payment collection, appointment scheduling, and cleanliness
Administer vaccinations and conduct health screenings
Educate patients on disease prevention and healthy lifestyles
Collaborate with healthcare providers, nurses, and medical assistants
Participate in community outreach activities
Participate in leadership-level planning for growth opportunities
Travel to community locations and work regularly on and around the mobile unit
Work in the brick-and-mortar clinic as needed when mobile clinic events are not scheduled

• Master's or DNP Family Nurse Practitioner (FNP) with completion of an accredited FNP program
• Current Family Nurse Practitioner (ANCC or AANPCB) Board Certification or equivalent certification
• Currently licensed or eligible for California APRN licensure as a Family Nurse Practitioner
• Clinical experience in family and primary care services preferred
• Experience with EHR preferred
• Bi-lingual preferred
• Willingness to provide world-class service to all stakeholders
• Ability to travel to various community locations, often in underserved or rural areas
• Ability to work some evenings or weekends, depending on the clinic schedule
• Ability to work in outdoor and varied weather conditions
