Community Support Facilitator

Hope Services is seeking a compassionate and driven Community Support Facilitator to join our dedicated team in the Bay Area. This role focuses on enhancing the lives of individuals with developmental disabilities through personalized care and fostering community engagement. As a facilitator, you will coordinate and deliver a variety of services, including vocational training, social integration, and independent living support. Responsibilities

• Develop, implement, and monitor individual service plans tailored to clients' needs and goals in adherence with Hope Services' standards.
• Act as a liaison between the clients, their families, community services, and care providers to ensure cohesive and coordinated support.
• Facilitate group sessions and one-on-one meetings to assess client progress and address any issues or concerns.
• Provide direct support and coaching in various environments including work, recreational, and educational settings.
• Maintain accurate and comprehensive client records and progress reports for review by management and relevant stakeholders.
• Organize and lead community integration activities that promote socialization and improve quality of life.
• Advocate for clients' rights and wellbeing within the community and assist in resolving any barriers to service.
