• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in clinical quality improvement projects.
• Exercise independent clinical judgment in patient management.

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of practice.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
• 2 years' experience in value-based care or managing high-acuity geriatric patients.
• Commitment to improving patient experience and outcomes.

Onsite Outpatient clinical setting in Gladstone, Missouri. Report to the Regional Medical Director. Standard schedule:
Monday–Friday, 8:00 AM – 5:00 PM.

Pay Range: $219,400 - $306,900 per year. This job is eligible for a bonus incentive plan. The incentive opportunity is based upon company and/or individual performance. The compensation range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, certifications, etc.

Center Well Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment, and more.
