MDS Coordinator – RN
We are looking for an experienced MDS Coordinator to manage MDS 3.0 processes and support nursing operations in a small skilled nursing unit within a continuing care retirement community.
ScheduleWeekdays with a predictable on‑call rotation and flexibility to balance hours during the week.
Sign‑on Bonus$5,000 sign‑on bonus.

- Timely completion of MDS 3.0 and submittals to CMS.
- Oversee the Care Plan initiation and Care Plan updating process.
- Assist in all phases of the delivery of nursing care for residents in the nursing facility.
- Initiate preventive and restorative nursing procedures for each resident to achieve the highest possible degree of function, self‑care, and independence based on resident choice, where practicable.
- Assist with assessing current residents to ensure proper level of care is provided and documented.
- Understand, practice, and communicate resident‑directed/centered care on a daily basis as it relates to the nursing department.
- Assist with providing orientation/training for staff.
- Assist and help develop nursing department continuing education, in‑services training and program development.
- Proficient with computers and have the ability to learn health information technology as it relates to nursing.
- Help support and manage all nursing/clinical aspects of state or federal government survey processes and regulations.
- Participate in on‑call rotation with other nurse leadership team members.
• MDS experience or willingness to attend MDS training and become RAC‑certified (company paid).
• RN licensure.
• Strong assessment, communication, and advocacy skills.
• Proficient with computers and ability to learn relevant health information technology.
