Seven Healthcare is proud to present a Travel Case Management RN opportunity in Glasgow, Kentucky, offering a weekly blended rate of $2,600 for five 8‑hour day shifts each week. This role is perfect for a detail‑oriented RN with strong care‑coordination skills and a passion for safe, effective discharge planning.

• Perform initial and ongoing patient assessments for discharge readiness
• Develop individualized care plans and coordinate post‑acute services
• Facilitate transitions to SNF, rehab, home health, outpatient therapy, or community programs
• Conduct utilization reviews and collaborate with insurance providers
• Educate patients and families about ongoing care needs and available resources
• Document all case‑management activities thoroughly and on time
• Serve as a patient advocate ensuring safe, appropriate, and efficient discharges

• Active Kentucky RN license (or compact eligibility)
• Current BLS certification
• Minimum 1–2 years of Case Management RN experience
• Strong interpersonal, time‑management, and communication skills
