MAJOR FUNCTION:
To provide palliative services in the hospital setting at IMC
- Mullica Hill. The practitioner will be assigned a specific patient load for which they will be responsible. This role will collaborate with our Palliative physicians to assess patients and provide palliative care to best treat our patients. APN will be responsible for developing and providing services for the Palliative Care program. In addition, will work collaboratively with physicians, nurses, case managers, clergy, hospice, and home care to meet the needs of patients with chronic progressive disease or terminal disease.
This role reports directly to the physician director for palliative care services LIFICATIONS:
Education & Experience:
MSN required.
Successful completion of advanced practitioner training and certification as a practitioner by an accredited nurse practitioner program and certifying body.
Minimum 3 years professional nursing (Nurse Practitioner or Clinical Nurse Specialist) in adult health and chronic disease management, pain management and palliative care.
Critical Care experience preferred.
Experience providing APN care for geriatric clients preferred.
Certification/Licensure:
Current APN and DEA license in NJ.RN license in NJ, ACLS preferred, and National Certification in area of specialty.

Define and document patient diagnoses
Assure all patients are completely satisfied
Patient and family education and management
