Job Summary
We are seeking a compassionate, dependable, and skilled Registered Nurse (RN) to join our skilled nursing facility team. The RN will provide high-quality nursing care to residents, promote their health and well-being, and work collaboratively with physicians, nursing staff, residents, families, and other members of the interdisciplinary care team.
The ideal candidate is a dedicated professional who demonstrates strong clinical judgment, excellent communication skills, and a commitment to providing person-centered care in a long-term care and rehabilitation setting.
ResponsibilitiesProvide direct nursing care to residents in accordance with physician orders, care plans, and facility policies.
Perform comprehensive nursing assessments and monitor residents for changes in condition.
Administer medications and treatments safely and accurately.
Monitor vital signs, pain levels, wounds, nutritional status, and other clinical indicators.
Recognize and promptly report changes in residents' conditions to the physician, supervisor, and appropriate members of the care team.
Develop, implement, and evaluate individualized care plans.
Provide wound care and other treatments as ordered.
Coordinate care with physicians, nurse practitioners, therapists, CNAs, LPNs/LVNs, and other healthcare professionals.
Communicate effectively with residents and their families regarding care, treatment, and changes in condition.
Accurately document assessments, interventions, medications, treatments, and resident responses in the medical record.
Supervise and provide direction to LPNs/LVNs and Certified Nursing Assistants (CNAs) as appropriate.
Participate in resident care conferences and interdisciplinary team meetings.
Assist with admissions, transfers, discharges, and hospital returns.
Follow infection prevention and control procedures and maintain compliance with facility, state, and federal regulations.
Respond appropriately to emergencies and initiate necessary interventions.
Promote resident dignity, privacy, independence, and quality of life.
Maintain a safe and supportive environment for residents and staff.
Perform other nursing duties as assigned.
Current and unrestricted Registered Nurse (RN) license in the applicable state.
Graduate of an accredited nursing program.
Current CPR/BLS certification, as required.
Previous experience in skilled nursing, long-term care, rehabilitation, or geriatric nursing preferred.
Strong clinical assessment and critical-thinking skills.
Excellent written and verbal communication skills.
Ability to work effectively as part of an interdisciplinary healthcare team.
Ability to prioritize multiple responsibilities in a fast-paced environment.

Ability to stand and walk for extended periods.
Ability to assist with resident transfers and repositioning as needed.
Ability to lift, move, and reposition residents in accordance with facility safety procedures.
Ability to respond quickly to resident needs and emergency situations.
